The Veterans Transition Center of California (VTC) aims to empower Veterans to move from crisis to self-sufficiency with the goal of achieving "Functional Zero." At VTC, we employ a Housing First model, ensuring immediate housing access without prerequisites to provide a stable foundation for rehabilitation and reintegration. We emphasize personalized case management, tailoring programs to address each Veteran's unique medical, psychological, financial, and social needs. Our holistic approach goes beyond housing, providing essentials like food, clothing, transportation, and specialized services including mental health counseling, substance abuse recovery, and employment resources. Since its inception, VTC has served over 20,000 Veterans.

Each Veteran’s journey with VTC is supported by personalized, client-centered case management, where our dedicated case managers develop tailored recovery programs addressing medical, psychological, financial, and social needs.
