Vets on Track Foundation is a Virginia-based nonprofit organization dedicated to ensuring that veterans, first responders, and their families remain honored, connected, and supported throughout every stage of life. We believe gratitude must be demonstrated through meaningful action—and that no one who served our nation or communities should ever feel forgotten, isolated, or left to face life’s challenges alone.
At the center of our mission is the Promise of Presence Program, a relationship-based outreach program created to address loneliness and social isolation among veterans and first responders residing in nursing homes, assisted-living communities, and other long-term-care settings. Many of these men and women once served on the front lines of our nation and communities, yet aging, illness, limited mobility, the loss of loved ones, and separation from familiar support networks can leave them increasingly disconnected. Their need for companionship, recognition, and belonging does not end when their active service or career does.
Through the Promise of Presence Program, trained volunteers and Program Ambassadors provide regular visits, meaningful companionship, attentive listening, encouragement, service recognition, and opportunities for continued community connection. These visits are intended to form genuine, consistent relationships—not simply provide one-time moments of appreciation. Volunteers may share conversation, participate in activities, listen to personal stories, recognize military or public-safety service, celebrate important milestones, and offer prayer or spiritual encouragement when requested.
The program also creates opportunities for veterans and first responders to remain engaged with the broader community. Working with care facilities, hospice organizations, veterans’ groups, first-responder agencies, businesses, faith communities, and civic organizations, the Foundation brings community support directly to individuals who may no longer be able to attend traditional events. Through these partnerships, we help ensure that physical limitations or residential placement do not result in social invisibility.
When a participating veteran or first responder transitions into hospice care, the Foundation’s Final Mile Initiative continues the Promise of Presence. Trained volunteers offer compassionate companionship, service recognition, quiet presence, and encouragement for families and caregivers. Depending on the individual’s wishes and circumstances, this support may include personal visits, veteran-to-veteran or first-responder peer companionship, recognition ceremonies, patriotic keepsakes, prayer or pastoral support, assistance preserving legacy stories, and final salutes or remembrance activities.
The Foundation does not provide medical care, clinical counseling, or hospice services. Our role is to work respectfully alongside professional care providers while offering the human connection that complements clinical care. Every visit is guided by the individual’s preferences, health needs, privacy, culture, faith, and family circumstances. Volunteers receive training in compassionate presence, appropriate boundaries, cultural awareness, trauma-informed companionship, privacy, grief, self-care, family support, and when to seek assistance from facility or hospice professionals.
The Promise of Presence Program is built upon a simple but powerful belief: presence matters. Consistent human connection can restore a sense of belonging, affirm personal dignity, and remind individuals that their service and life stories continue to have value. For families, knowing that a trusted volunteer is visiting and honoring their loved one can also provide reassurance, encouragement, and an added sense of community support.
The Foundation seeks to build a dependable network of volunteers and community partners capable of extending this program throughout Central and Northern Virginia and, over time, into additional communities across the Commonwealth. This work includes recruiting, screening, and training Program Ambassadors; establishing partnerships with nursing homes, assisted-living communities, and hospice providers; coordinating individual and facility assignments; providing recognition materials and program resources; supporting volunteers; and measuring the consistency and impact of each relationship.
In addition to the Promise of Presence Program, Vets on Track Foundation strengthens veterans, first responders, and their families through initiatives centered on remembrance, resiliency, service, and community connection. The Fallen Heroes Initiative honors those who made the ultimate sacrifice by supporting their surviving families and helping provide educational opportunities for their dependents. Virginians Give Back mobilizes individuals, businesses, and communities to express appreciation through service and outreach. Resiliency Among Warriors promotes peer connection, encouragement, and access to resources that strengthen emotional, social, and spiritual resilience. The Community Connection Project creates recurring opportunities to gather and build relationships through monthly Coffee Socials across the Richmond region, Flag Retirement Ceremonies, Community BBQs, and other welcoming events designed to reduce isolation and strengthen belonging.
Together, these programs create a continuum of support that honors sacrifice, strengthens resilience, builds relationships, and preserves dignity. Whether gathering around a table at a Coffee Social, recognizing a resident's service in long-term care, supporting a family after loss, or sitting quietly beside someone receiving hospice care, Vets on Track Foundation turns gratitude into action through personal presence.
Our work is made possible by volunteers, donors, businesses, care providers, civic organizations, faith communities, and other partners who believe that honoring service is a continuing responsibility. Their support allows the Foundation to reach individuals who might otherwise remain isolated, build sustainable community partnerships, equip volunteers to serve responsibly, and ensure that veterans and first responders receive consistent recognition and companionship.
Our promise is straightforward and enduring: we will show up, listen, honor, and remain present. From the challenges of aging and isolation through the final mile, Vets on Track Foundation is committed to ensuring that veterans and first responders know they are remembered, their sacrifices still matter, and they remain part of a grateful and caring community.
