Hearts for the Invisible Charlotte Coalition (HFTICC) is a nonprofit organization dedicated to eradicating housing inequities for people experiencing homelessness throughout Mecklenburg County, North Carolina. Founded on the belief that every person deserves dignity, safety, and a place to call home, HFTICC serves individuals and families who are unsheltered and living in encampments, vehicles, abandoned buildings, wooded areas, or other places not meant for human habitation. Through compassionate, trauma-informed, and housing-focused outreach, HFTICC works to ensure that our most vulnerable neighbors are not invisible, but seen, supported, and empowered on their path toward stability and permanent housing.
HFTICC is the lead street outreach agency for Mecklenburg County and provides direct outreach and housing-focused case management services across Charlotte and surrounding communities. Our team meets people where they are—physically, emotionally, and socially—to build trust and connect them with life-saving resources and long-term solutions. We understand that homelessness is complex and often rooted in systemic inequities, trauma, economic instability, health challenges, and barriers to affordable housing. Because of this, our work centers dignity, equity, and individualized support rather than judgment or one-size-fits-all approaches.
Using a housing-first model, HFTICC prioritizes helping individuals access stable housing as quickly as possible while also addressing the barriers that may prevent them from maintaining housing long term. We believe housing is a basic human right and that people are more successful in addressing healthcare, employment, behavioral health, and other needs when they have a safe and stable place to live. Our services are designed to reduce barriers and create pathways to permanent housing and improved quality of life.
HFTICC provides a broad range of services to individuals experiencing homelessness, including street outreach, coordinated entry navigation, housing-focused service planning, emergency shelter connections, transportation assistance, document and ID recovery, landlord and housing provider communication, healthcare and behavioral health referrals, benefits enrollment assistance, and ongoing follow-up support. Outreach teams also distribute food, water, hygiene supplies, blankets, weather-related survival items, and other necessities to individuals living outdoors. By combining immediate survival support with long-term housing navigation, HFTICC helps bridge the gap between crisis and stability.
HFTICC also partners with other agencies for Wellness Without Walls, a Street Psychiatry Program, which brings behavioral health support directly to individuals living unsheltered. Through this program, licensed clinical professionals and psychiatric providers engage individuals in the community who may otherwise face significant barriers to accessing mental health care. Wellness Without Walls helps connect participants to psychiatric evaluations, medication management, behavioral health treatment, crisis intervention, and ongoing mental health support while meeting individuals in environments where they feel safest and most comfortable. This innovative approach reduces barriers to care, builds trust with highly vulnerable populations, and improves outcomes for individuals experiencing severe mental illness, substance use challenges, and chronic homelessness.
In addition to direct service delivery, HFTICC plays an active role in strengthening community partnerships and improving systems of care for individuals experiencing homelessness. We collaborate closely with local governments, healthcare providers, housing agencies, shelters, nonprofit organizations, faith communities, and community advocates to improve coordination and create more equitable access to resources. Our team works to identify service gaps, advocate for housing solutions, and elevate the voices and lived experiences of people directly impacted by homelessness.
HFTICC’s approach is deeply rooted in trauma-informed care and relationship-building. Many individuals experiencing unsheltered homelessness have experienced significant trauma, discrimination, or systemic exclusion, which can create distrust of traditional systems and institutions. Our outreach teams prioritize consistency, empathy, and respect, recognizing that meaningful change often begins with human connection. We strive to create safe and supportive relationships that empower individuals to make informed decisions about their goals and future.
Our work also emphasizes equity and inclusion. Homelessness disproportionately impacts marginalized populations, including people of color, LGBTQ+ individuals, veterans, survivors of domestic violence, and individuals living with mental health or substance use challenges. HFTICC is committed to addressing disparities in access to housing and services and ensuring that all individuals are treated with dignity and compassion regardless of their background or circumstances.
The impact of HFTICC’s work is measurable and community-driven. In 2025, HFTICC served 755 individuals experiencing homelessness throughout Mecklenburg County, achieved 363 positive exits from homelessness, and helped secure 198 permanent housing placements. Through consistent street outreach, case management, housing navigation, and behavioral health support, HFTICC continues to provide critical interventions that improve stability, health outcomes, and long-term housing success for individuals and families experiencing homelessness.
Financial support and community partnerships are essential to sustaining and expanding HFTICC’s impact. Donations and grants support critical outreach and housing-focused case management services, emergency survival supplies, transportation assistance, housing barrier removal costs, identification recovery, behavioral health and street psychiatry services, staff training, data coordination, and community collaboration efforts. Flexible funding allows HFTICC to respond quickly and compassionately to urgent needs while also investing in long-term solutions that reduce homelessness across the region.
At its core, Hearts for the Invisible Charlotte Coalition exists to ensure that no person experiencing homelessness is overlooked or forgotten. We believe that communities are strongest when every person has the opportunity to live safely, with dignity, stability, and hope. Through outreach, advocacy, behavioral health support, and housing-focused services, HFTICC continues working toward a future where homelessness in Charlotte-Mecklenburg is rare, brief, and nonrecurring.
