On January 1, 2024, Family & Community Services merged with AxessPointe
Community Health Centers, forming Axess Family Services (AFS).
This partnership strengthens access to comprehensive healthcare, behavioral
health services, social services across Summit, Portage, Trumbull, and Mahoning
counties, and a wide range of social services throughout Northeast Ohio, and parts
of Pennsylvania and Michigan.
Together, Axess Family Services is committed to a person-centered approach to
health and well-being, ensuring that individuals receive the care they need, both
medically and socially, advancing health equity for all.
