Proposal for One-Year HIV/AIDS Educational Resource Program

, MOMMY & DADDYS LITTLE ANGELS YOUTH & FAMILY SERVICES INC

Mommy & Daddy's Little Angels Youth and Family Services Inc., I am pleased to submit this proposal for a $500,000 grant to launch and sustain a vital one-year HIV/AIDS Educational Resource Program aimed at young adults in underserved urban communities. Our goal is to provide critical education, advocacy, and resource navigation services to improve the health and overall well-being of those living with or at risk for HIV/AIDS.
Organizational Background
Mommy & Daddy's Little Angels Youth and Family Services Inc. is a 501(c)(3) nonprofit supporting disadvantaged youth and families in New York City’s urban communities. Since 2010, we have provided essential resources such as education, healthcare access, clothing, and hygiene products, with a particular focus on individuals impacted by HIV/AIDS and heart conditions. We aim to foster resilience and equitable opportunities for those who most need it.
Why This Program is Critically Needed
Young adults living with HIV/AIDS face a multitude of challenges that extend far beyond their diagnosis. These individuals are often burdened by social stigma, a lack of education on how to manage their health effectively, and barriers to accessing medical care. Furthermore, financial instability and difficulty navigating complex healthcare systems exacerbate their challenges, leaving many feelings isolated, overwhelmed, and without direction.
The healthcare system can often feel like a maze, challenging to navigate without guidance, and this complexity is particularly overwhelming for young adults who may not have the resources or support systems to advocate for their care. Unfortunately, this lack of consistent follow-up and case management support results in gaps in care, where individuals do not receive the ongoing attention and resources, they need to take control of their health and well-being.
The Detailed Budget and Financial Plan allocates funds strategically to ensure the program's success. Equipment & Technology Costs ($100,000) will cover essential purchases, including PC desktops ($20,000 for 10 units), laptops ($25,000 for 10 units), phones ($10,000 for 10 units), tables and chairs ($5,000), printers and scanners ($10,000), fax machines ($2,000), and software and online service subscriptions ($25,000) for platforms like Zoom, health tracking tools, and office software. Space & Rent ($60,000) will fund a 12-month facility lease at $5,000 per month, providing a dedicated space for educational sessions and in-person participant support. Supplies ($45,000) will ensure smooth operations, including office supplies ($5,000), educational materials such as books and guides ($15,000), health resources and wellness kits ($10,000), program supplies like notebooks and pens ($5,000), and miscellaneous needs ($10,000). Staff Training and Development ($30,000) will invest in staff through training programs on health education, customer service, HIV/AIDS awareness, and program management. Staff Salaries and Stipends ($40,000) will ensure the program is staffed by dedicated professionals, covering a Program Manager's salary ($25,000) and stipends for five program staff members ($15,000). To raise awareness, Marketing & Outreach ($25,000) will support website development and maintenance ($5,000), social media campaigns ($10,000), printed flyers and posters ($5,000), and community outreach events ($5,000). Ensuring program effectiveness, Evaluation & Monitoring ($25,000) will fund data collection tools ($5,000), surveys and feedback mechanisms ($5,000), external evaluator fees ($10,000), and monitoring staff/consultants ($5,000). Lastly, a Contingency Fund ($15,000) is allocated to address unexpected costs or overages that may arise during the first year. Partnering with us and believing in our mission will have a profound, lasting impact on the lives of young adults living with HIV/AIDS. By joining hands with our organization, you will become an integral part of a transformative initiative that empowers individuals who have long been overlooked or underserved. Our program is designed to provide education, support, and resources these young adults need to manage their health and build a better future.
Enhancing Training for Participants and Service Providers
To ensure lasting impact and empowerment, we will offer specialized training for both young adults in the program and service providers. Our approach centers on educating participants about the critical resources available and providing hands-on assistance in applying for and navigating these services. Many of the services these individuals need are already available, but they often go unnoticed due to a lack of outreach, information, and guidance. Without proper education, individuals may remain unaware of the vital support systems that can significantly improve their quality of life.
1. Training for Young Adults in the Program
We will provide targeted education and practical application support to ensure young adults can confidently access essential services. Our training will include:
o plan and the significance of adhering to prescribed regimens.
o Practical support: Assisting in accessing regular testing, managing appointments, and ensuring they have access to necessary medications and support services.
• Navigating government services and social programs effectively:
• Reducing stigma through education and community support:
o Workshops on stigma: We will provide educational workshops aimed at reducing HIV-related stigma, mental health stigma, and general discrimination.
o Peer support networks: Connecting participants to peer groups and mentorship opportunities, where they can share experiences and receive guidance from individuals who have navigated similar challenges.
o Community outreach and awareness: Teaching participants how to educate their communities and create a culture of acceptance and support.
• Practicing self-care and mental health management:
• Learning how to apply for financial assistance, secure stable housing, and managing household responsibilities:
2. The Challenge: Lack of Awareness and Access to Resources
This program will bridge that gap, ensuring that every participant understands how to access the services that can improve their quality of life.
