In partnership with MUSC Cares Clinic and Charleston Mental Health, we embed licensed social workers into care delivery—ensuring that trauma, untreated illness, and emotional distress are addressed alongside housing and financial needs.
From a mother battling depression while raising children in a hotel, to a working father who can’t afford his medication and is facing utility shutoff—our team is there to intervene early, walk with them through the crisis, and help them build a path forward.
Our Services Include:
Onsite medical care (MUSC Cares)
Mental health coordination (Charleston Mental Health)
Housing and eviction prevention
Utility and emergency financial assistance
Placement into emergency shelter and permanent housing
Coordination of treatment for addiction and mental health
School-based support in partnership with CCSD
Street outreach and daily meals through faith-based partners
