Purpose of Grant Request - Conduct Homeless Veteran Outreach in South Carolina. The Veteran Connect Project finds its purpose in connecting Homeless Veterans with programs, services, and resources. The overall goal is to engage Homeless Veterans, assess their needs, and connect them with services to fill their needs. We believe when needs are filled, it not only increases quality of life but also promotes homeless recovery.
Action - We will engage Homeless Veterans where they are (streets and shelters), conduct needs assessments (with the Veteran’s consent), and produce/facilitate referrals direct to service providers. Inspire Real Change currently has a network of 50+ Veteran service programs and resources within the Columbia/Richland County area of South Carolina. Resources include the Dept. of Veteran Affairs/Columbia Healthcare System, Veteran Service Offices, and Veteran Transitional Housing.
Strategy – Our team will engage and assess Homeless Veterans through a Triage Model approach, which is integrated with a Trauma-Informed Quality Care Model. This model prioritizes needs and coordinates referrals through a process that is Client driven/motivated and care accountable within the network of service providers. We will assist and navigate Veterans through the service network, conduct follow-up assessments, and evaluate outcomes and/or redirect services.
Anticipated Outcome – Engage/Connect a minimum of 25 Homeless Veterans and Achieve an 85% Goal Completion/Needs Met success rate across all Veteran participants served (85% Successful Outcomes).
