Our goal is to assist those in crisis and/or a cycle of poverty with direct, practical compassion as well as connection with resources to provide stability and opportunity for future progress and economic and social stability.
We connect volunteers with local non-profit and community volunteer needs through our website (SOS platform), monthly newsletter, FaceBook and Instagram page, and during Community Meetings we host. For a deeper volunteer experience we provide a Care Advocacy Training to equip volunteers to walk alongside of participants exiting out of care coordination for additional ongoing support and meaningful connection.
Our care coordination model "The Care Wheel" identifies urgent family/individual needs re: homelessness prevention such as emergency shelter and rental assistance to avoid eviction, payment of delinquent utility bills, assistance with car repair, matching vehicle donations with those in our communities in need of transportation, providing local food, clothing resources, connecting individuals with local job search resources, mental health assessments and reconnecting participants with family members. Over 80% of our clients are in the federally defined extremely low income category, approximately 37% are homeless and 33% female head of household (primarily single mothers with children). Our largest referral source is the local school districts' Child Welfare and Attendance Specialists and Social Workers. We also receive referrals from staff at local cities, churches, businesses, and the community in general.
We find needs; needs find us. We provide customized care coordination with the goal of encouragement and solutions to address individual/family human service needs in the Tri-Valley.
