We design our services with the individual family in mind, to meet both the needs of individuals diagnosed with Alzheimer’s disease and related dementia and those caring for them. We strive to support each individual’s needs while also supporting strengths, abilities, and independence. We offer the following services:
Adult Day Services which provide structured, person-centered, and therapeutic activities to adults experiencing cognitive impairment related to chronic conditions such as Alzheimer’s disease and other dementia and respite for family caregivers. Our professional staff deliver services in a safe and caring environment. Our services include assistance with personal needs, group and individualized activities, light breakfast, lunch, and snacks, medication reminders, therapeutic music programs, physical exercise programs, care management, nursing and social work services, and transportation to and from the center. The day center operates on a voluntary cost-share contribution and additional assistance may be available through PASSPORT and MyCare Ohio programs and the Department of Veteran’s Affairs.
Short-term Institutional Respite Care Program offered in collaboration with the Area Office on Aging of Northwestern Ohio’s Caregiver Support Program, provides short-term respite care to family caregivers by paying the cost for room and board in a nursing home or assisted living facility.
Caregiver Support Supplemental Services offered in collaboration with the Area Office on Aging of Northwestern Ohio’s Caregiver Support Program, this program helps family caregivers with acquiring and paying for resources and materials to help with caregiving responsibilities.
Website, Information Materials, and Education information and education are available through a variety of avenues including a website, over the telephone, face to face meetings, and group programs. These services area free and focus on dementia and caregiving. Printed, current informational materials about the dementia, caregiving, and local community resources and educational and outreach activities for the community are also available free of charge.
