Partners for HOME serves as the Collaborative Applicant for the Atlanta Continuum of Care (CoC) — a HUD program that promotes community-wide commitment to the goal of ending homelessness and provides funding for efforts by nonprofit providers and state and local governments to quickly rehouse people impacted by homelessness. While not a direct service provider, Partners for HOME provides the following services within the City of Atlanta:
• Homeless Response: Partners for HOME provides thought leadership, coordination and management of housing interventions, services and resources, and special populations and policies that impact the homeless ecosystem. This includes strategic guidance around housing interventions (e.g. permanent supportive housing, rapid rehousing, diversion); service needs (i.e. street outreach, coordinated entry, case management and health care); trainings for service providers working with special populations (e.g. domestic violence, severe and mental illness, youth, veterans, LGBTQ+); and leadership in advocacy and policy efforts related to homelessness.
• Performance and Compliance Management: Partners for HOME measures and evaluates the performance of direct service providers to ensure they are reaching benchmarks and goals that will improve efforts to end homelessness in the City of Atlanta.
• Fundraising and Grantmaking: Partners for HOME secures and administers government and private funding to implement solutions that support strategic, citywide efforts to end homelessness in the City.
Collectively, these offerings create a streamlined and coordinated ecosystem for stakeholders in the homeless space. Funding partners can direct monies toward system-level work to end homelessness — as opposed to helping a single, targeted population or effort — and providers have access to collective tools and resources to improve outcomes and reduce homelessness within their targeted populations. Using a strategic and coordinated approach to ending homelessness, we ensure stakeholders are working toward a shared goal of ending homelessness across the City.
