Palmetto Community Health Care Center

Palmetto Community Health Care (PCHC) is a nonprofit organization based in Rock Hill, South Carolina, dedicated to providing compassionate, high-quality medical services at no cost to uninsured adults aged 18 to 64 in York and Lancaster Counties. Our mission focuses on promoting health equity by ensuring access to essential healthcare for individuals lacking medical coverage.
PCHC offers a comprehensive range of health services, including:
-Management of chronic health conditions such as high cholesterol, diabetes,
gastrointestinal issues, hypertension, obesity, and thyroid disorders.
-Treatment for minor acute illnesses like bronchitis, sinus infections, and influenza.
-Primary care services.
-Onsite screenings for breast, cervical, lung, and colon cancers.
-Nutritional counseling.
-Optometry and eye care.
-Mental Health Counseling off-site
-Routine lab and Prescription medications with our local partners

To qualify for services, individuals must be residents of Lancaster or York County, aged 18 to 64, without health insurance coverage or veteran’s benefits, not covered by Medicaid or Medicare, and have a household income up to 250% of the Federal Poverty Level. PCHC's history reflects a community-driven effort to improve lives, renew hope, and manage chronic diseases. Over the past 16 years, the organization has evolved to provide quality healthcare to underserved adults, offering foundational strategies and resources for living a healthy life and managing health issues.

In recognition of their commitment to quality care, PCHC earned a 2023 Gold Rating from the National Association of Free & Charitable Clinics (NAFC) Quality Standards Program.

PCHC is located at 410 Oakland Ave., Rock Hill, SC 29730. Office hours are Monday through Thursday, 8:00 am to 12:00 pm and 1:00 pm to 5:00 pm; the office is closed on Fridays.
