Continuum of Care, Inc. (COC) is a leading nonprofit organization transforming the lives of adults facing some of life’s toughest challenges—serious mental illness, substance use disorders, and severe developmental or intellectual disabilities such as Autism, traumatic brain injury, and seizure disorders.
From severe depression and schizophrenia to PTSD and bipolar disorder, the people we serve are often those society overlooks. Nearly 95% arrive at our doors homeless, chronically hospitalized, or recently institutionalized—without stability, support, or hope.
At Continuum, we meet them where they are—and help them build a new life.
With over 60 properties across Connecticut, including group homes, crisis centers, and extended living facilities, we are one of the state’s largest community housing providers for individuals with complex behavioral and developmental health needs. We serve more than 2,500 individuals every year—connecting them to safe housing, critical treatment, and the tools to lead independent, fulfilling lives in the community.
For 60 years, Continuum has been a catalyst for lasting change. We’ve reunited families, revitalized neighborhoods, saved taxpayers millions, and restored dignity and purpose to those who had nowhere else to turn.
Our comprehensive, person-centered approach delivers proven results—for the individuals we support, for their families, and for the broader community. We don’t just provide services. We rebuild lives.
Continuum is a trailblazer in residential and crisis care—constantly pushing boundaries to serve individuals with the most complex and urgent needs.
Over three decades ago, we opened the nation’s first community-based crisis center in a residential setting, breaking away from institutional models and proving that compassionate, community-centered care saves lives.
We serve those others won’t—individuals with suicidal ideation, dual diagnoses, and high-risk behaviors. Where others see risk, we see potential.
Our pioneering spirit continues to shape the field:
We created the first community-based residential treatment program for veterans in the U.S. not operated by a hospital—offering a healing environment with dignity and support.
We launched Connecticut’s first longer-term, non-congregate emergency housing shelter for homeless individuals—prioritizing privacy, safety, and long-term stability.
We established Connecticut’s first group home specializing in Dialectical Behavior Therapy (DBT)—offering targeted, evidence-based support for individuals with severe emotional dysregulation.
Every innovation stems from our belief that no one is beyond hope, and that the right support, at the right time, can transform a life.
Continuum of Care: Leaders in compassion. Experts in change. Champions of possibility.
