ELDERBRIDGE AGENCY ON AGING

Elderbridge services include Case Management, Elder Rights, Family Caregiver Services, Nutrition Services, and more. Elderbridge provides either direct services to clients or contracts with Home and Community-Based Services (HCBS) providers to deliver these services.

The Elderbridge rural region covers 16,190 square miles. We are the largest aging organization in the state of Iowa. Our 29 County area includes Audubon, Buena Vista, Calhoun, Carroll, Cerro Gordo, Clay, Crawford, Dickinson, Emmet, Floyd, Franklin, Greene, Guthrie, Hamilton, Hancock, Humboldt, Kossuth, Lyon, Mitchell, O’Brien, Osceola, Palo Alto, Pocahontas, Sac, Sioux, Webster, Winnebago, Worth, and Wright Counties.

Direct services are provided in the client's home, at one of our Elderbridge offices located in Carroll, Fort Dodge, Mason City, or Spencer, or at one of our 52 meal sites scattered across our 29-county region.

In FY24, Elderbridge served 37,179 individuals, including older adults aged 60 and above, adults with disabilities aged 18-59, and caregivers.
