Carry-Out Caravan delivers weekly groceries providing nutrition and breaking isolation for those who are homebound; Fix-It provides fall prevention and minor home repairs; YardBusters engages in annual yard cleanup; SnowBusters prevents falls by conducting snow removal; and Medical Mobility and Veterans Medical Mobility provide escorted rides to medical appointments. In addition, Cultivate distributes 3,500 File of Life pockets, mobilizes 600 RSVP volunteers and provides resources and referrals for more than 1,500 callers annually. Because of Cultivate, our clients can stay in their homes with a sense of comfort and independence!
Cultivate provides access to health care, fall-prevention, home safety measures, nutrition, and companionship. Current programs include:
● Medical Mobility – Provides volunteer-escorted medical rides for adults age 60+. The Boulder County Area Agency on Aging (BCAAA) reports that nearly 60% of seniors in Boulder County consider their physical health to be a problem in their life. Transportation is a linchpin issue that has significant bearing on the ability to age in place. Medical Mobility was named Denver Regional Mobility and Access Council member of the year in 2013 for outstanding leadership in promoting transportation mobility and access.
● Fix-It – Provides fall prevention and minor home repairs for adults age 60+. BCAAA reports that approximately 40% of Boulder County seniors need help cleaning up their yard and/or home.
● YardBusters - Provides annual yard cleanup to adults age 60+. BCAAA reports that approximately 40% of Boulder County seniors need help maintaining their yard and/or home.
● SnowBusters - Provides fall prevention through snow removal to adults age 60+. BCAAA reports that approximately 40% of Boulder County seniors need help maintaining their yard and/or home.
● Carry-Out Caravan – Provides grocery shopping and delivery to adults age 60+. According to BCAAA, 32% of seniors in Boulder County feel they have trouble maintaining a healthy diet. Proper nutrition among older adults can help maintain health and independence, delay or prevent institutionalization, delay morbidity and mortality, improve quality of life and reduce health care costs associated with aging and chronic diseases.
Community Programs: Cultivate provides information and education to seniors and caregivers throughout Boulder County with these services:
● RSVP 55+ Volunteer Program – Mobilizes volunteers ages 55+ to provide meaningful contributions to the community.
● File of Life – A national distribution program of medical record pockets to reduce response time in an emergency for first responders and hospital staff. Annually, Cultivate partners with local hospitals and city officials to print and distribute 3,500 Bilingual File of Life pockets at no cost to clients.
● Resources and Referrals – Cultivate utilizes strong relationships with local non-profits and government agencies through cross referrals and partnerships to provide information for more than 1,500 callers annually. Cultivate staff provides regular presentations throughout the community to inform and educate the public.
