The Veteran Lifeline Initiative is an ongoing program by Helping Hands Bridging Gaps, a 501(c)(3) nonprofit (EIN 41-2883449) founded in 2024 in Surprise, Arizona. Designed to reduce veteran homelessness and empower Arizona's West Valley veterans, the initiative provides year-round, hands-on support through monthly rescues, personalized advocacy, and resource connections.
Key Components:
Monthly Rescues: Direct aid to 20-25 veterans per month, including feeding (gift cards/meals), clothing/hygiene kits, and temporary housing (hotel vouchers for 3-5 nights) to stabilize immediate needs.
Advocacy and Navigation: Benefits assistance, such as VA claims filing, mental health referrals, and insurance coordination—drawing from founder Azure Farrell's experience as a client advocate at CVS Health Aetna via Booz Allen Hamilton.
Resource Connections: Job placements, transportation to appointments/referral centers (e.g., rideshares/gas reimbursements), and long-term housing transitions.
Community Events: Quarterly workshops and job fairs to build networks and prevent recurring homelessness.
Goals and Impact:
Aim to support 200-300 veterans annually, contributing to Arizona's 8% yearly reduction in veteran homelessness (based on 2025 Maricopa County PIT Count: ~1,208 homeless veterans county-wide). Focus on measurable outcomes like 75% permanent housing exits and 55% employment at discharge, tracked via software and quarterly reports.
Unique Value:
Unlike centralized VA programs, our mobile, this is a driven approach that fills gaps in immediate, street-level intervention ensuring benefits work for veterans, not the other way around.
