PATHWAYS TO CARE INC

Helping Central Florida’s vulnerable homeless population of men and women, many of whom are veterans, in need of a safe place to heal from serious illness or injury.

Pathways to Care (PTC) opened in 2003 as a not-for-profit licensed Assisted Living Facility that serves the medically needy homeless by providing essential services with loving care. PTC serves as a post hospital resource, an excellent discharge solution. In 2015, we expanded our services to include a bridge housing option called Pathways Step2.

We collaborate with hospital systems, Veteran’s Administration, Housing Opportunities for Persons With AIDS network (HOPWA), faith-based organizations, local governments and businesses, community agencies and other medical entities.
