Our care teams work closely with the patient’s personal physician and/or Halifax Health - Hospice physician to develop a personalized plan of care to manage pain, control symptoms and ensure physical comfort so our patients can focus on enjoying this precious time with their loved ones.
Our extraordinary range of care encompasses many services, which include, but are not limited to:
• Full-time Medical Director who is board certified in hospice and palliative medicine. We are the only hospice program in our service area, and one of only a few in the nation, to do so.
• In-home care from a specialized, interdisciplinary team of professionals who are available to meet your needs 24 hours a day, seven days a week.
• Licensed Clinical Social Workers who help support the psychosocial and emotional needs of the patient and family, In addition, they assist in obtaining needed community resources.
• Spiritual support with ordained master’s level clergy.
• Children’s grief counseling at our BeginAgain Grief Centers located in Volusia and Flagler counties.
• Offers individual and group grief counseling with master’s level social workers and counselors.
• Specialized grief counseling for families and loved ones who have experienced a traumatic loss such as those related to war, suicide or violent crime.
• Veteran’s Recognition Program ensuring that our country’s heroes receive the honor and recognition they deserve, even if it comes near the end-of-life, through our pinning ceremonies.
• Complimentary therapies including massage, clinical aromatherapy, healing touch, guided imagery and pet visitation that help to relieve stress for the caregivers and bring additional comfort to our patients.
There is no place like home – or a home away from home.
While many Hospice patients choose to receive care in their own homes, others find comfort in our free-standing Halifax Health - Hospice Care Centers in Port Orange, Orange City, Ormond Beach and Edgewater. In all four locations, our patients receive loving personalized care from an expert team of physicians, registered nurses, licensed clinical social workers, spiritual care chaplains, certified nursing assistants and volunteers. Our private patient suites provide a home-like atmosphere, and common areas offer additional space for family and friends to gather.
Above all, our Care Centers provide a serene and comfortable place where privacy and human dignity are honored and a patient’s final days can be lived out in the warm embrace of family and friends. Loved ones, including pets, are welcome to stay overnight and each patient suite provides a pull-out sofa or sleeping chair to accommodate visitors.
Care comes in many forms:
Care in the Home - Enables patients to stay at home during the final months of life. This is the primary goal of hospice care.
Care in the Nursing Home or Assisted Living Facility - Allows Halifax Health - Hospice staff to work closely with facility staff to coordinate a plan of care that is appropriate to each patient and family.
General Inpatient Care - Provides a higher level of skilled care in the nursing, hospital or at a Halifax Health - Hospice Care Center during periods of crisis.
Respite Care - Provides short-stay comprehensive care at a Halifax Health - Hospice Care Center or other contracted facility, to assist caregivers when they need temporary relief from caring for their loved-one.
