Our Philosophy and Approach:
At H4H, we believe the root of the homelessness crisis in San Diego is chronic disconnection as a result of accumulated traumatic experiences and adverse social and economic conditions. To meet the critical needs of our unhoused neighbors, we follow a holistic approach. We work in partnership to restore the impacted individual or family’s sense of dignity, autonomy, and competence and their connection to community, hope, and a thriving future.
We raise awareness for the needs of our houseless neighbors to the San Diego community at large, to restore our communal responsibility to offer mutual support and aid to those who are in need.
We honor, value, and engage the unique wisdom and expertise of those who have experienced homelessness by hiring them in leadership positions or serving on our board.
Our evidenced-based services and approach have been collaboratively designed by listening deeply to the needs of those we serve with an awareness and respect for their right to personal choice and responsibility.
We focus our efforts on areas with known concentrations of individuals experiencing homelessness and build collaborative relationships with partnered organizations to minimize red tape and complicated processes that create barriers for people quickly being housed.
We fill the gaps in services available by providing immediate and afterhours intervention and problem solving services.
We cultivate a strong foundation of resources based on personalized guidance, and the patient cultivation of high quality relationships built on trust and mutual partnership enabling and promoting the self-determination of those we serve.
Through a community based and individualized/self-directed approach we provide compassionate care and advocacy to chronically homeless and medically fragile seniors, emergency supportive housing, and Shared-Living Housing options which support individuals and families in progressing towards permanent housing and Self-Sufficiency through 3 phases of movement- with specific services being provided informed by the specific needs, agreement, and direction of those being served. These Phases of Mutual Support include:

Phase 3: Self-Sustaining and Self-Contributing
Our flexible and individualized approach has resulted in our success in housing over 120 San Diego residents in 2 years with only a limited staff of 2 dedicated volunteer workers!
Individualized Programs and Services:
Emergency Shelter and Supportive Services Program serves seniors who are chronically homeless, medically fragile, and high users of emergency service users in San Diego County by providing:
Private hotel room for participants to stabilize and heal while experiencing a medical emergency
Onsite case management, Housing navigation, and Lived Experience Coaching with the goal of connecting participants to permanent housing, their support system, and existing services that have often lapsed due to difficulty of providers making contact with participants.
Assistance with securing vital documents such as ID, Social Security Card, and Birth Certificate
Regular case conferencing, collaboration, and coordination of services with community partners which eliminates red tape and complicated processes and systems which often discourage already overwhelmed seniors
