Founded in 2023, the Uplifme Resource Hub (URH) works at the intersection of poverty and health by addressing the social determinants of health. Uplifme provides real time information and connection to services addressing the social determinants, in a one stop shop, easy to use website, benefitting low income individuals, families and communities. The information found on the URH addresses both the physical and behavioral social determinants of health and is a tool that can increase health literacy among users.
The site includes informational flyers on resources provided by community based organizations, hospitals, government agencies and more, that offer services across a range of needs, to the public. Services and resources include information on job training programs, academic and vocational scholarships, hunger relief efforts, chronic disease management and available medical care to name a few. The site also includes a library of resources hubs across the county, addressing a range of populations including seniors, veterans, lgbtqia persons, and a range of issues including black maternal health and disease management disproportionately affecting disenfranchised communities. The library also includes blogs, vlogs and easy to use infographics that can increase health literacy amongst users. Finally, users will have the unique opportunity to connect to Community Health Workers (CHW) to assist in navigating information and directly connecting people to resources and services.
Overall, the URH site is intended to help people find and connect to all available supports and services in their communities easily, to address needs and improve the quality of their lives.
