Core Life Link serves individuals whose health is affected not only by disease but also by the conditions in which they live. The community includes residents who are uninsured or underinsured, people living with or at risk for HIV, STIs, or Hepatitis C, individuals with chronic health needs, people experiencing homelessness or housing instability, low-income families, veterans, people affected by substance use or behavioral health challenges, and residents who face transportation, employment, educational, language, or health-literacy barriers.
These barriers reinforce one another. Food insecurity can make medication adherence difficult and can worsen chronic illness. Housing instability can disrupt treatment, storage of medication, communication with providers, and personal safety. Lack of transportation contributes to missed appointments, delayed testing, and incomplete follow-up.
Unemployment and limited education may reduce access to insurance, stable housing, and reliable income. Veterans may encounter additional challenges related to disability, trauma, benefits navigation, isolation, or difficulty coordinating services across systems.
Individuals affected by HIV, STIs, and Hepatitis C may also experience stigma, fear, confidentiality concerns, or distrust that delays testing and treatment. When preventive services are difficult to access, conditions may remain undiagnosed and transmission can continue. When people are diagnosed but not successfully linked to care, the benefit of testing is limited. Core Life Link therefore emphasizes confidential, respectful care, prompt linkage, clear education, medication access, and ongoing navigation.
The need is also organizational. Communities often have valuable resources, but those resources may be dispersed across agencies with different eligibility requirements, schedules, locations, and documentation standards. A person in crisis may be given several phone numbers but no direct assistance completing the next step. Core Life Link addresses this gap by helping clients prioritize needs, understand referrals, gather required information, and connect with the appropriate provider or agency.
